Our neurorehab occupational therapists will assess and evaluate your needs relating to activities of daily living, functional status, fine and gross motor coordination, home safety and adaptive equipment, as well as other needs of the home and community environments.
Through assessment and evaluation, we can help you relearn daily living skills, manage your home environment, and adapt your environment and equipment to meet your changing needs.

Recreation and Life Skills Therapists are essential in returning clients to independence, getting them back to what they enjoyed prior to their injuries. They assess and evaluate the client’s needs relating to personal recreation and leisure interests, community support and resources, home management, and health and wellness for overall improved physical, mental, cognitive, and emotional wellbeing.
Recreation and Life Skills Therapists tailor their focus on community reintegration, transportation access, safety awareness, time management, and other community support opportunities.

Our neurorehab speech and language pathologists can address communication difficulties you may experience. We can assess and evaluate your needs relating to communication, cognition, swallowing impairments, functional status, home and community safety, and adaptive equipment, to help you improve your communication skills and regain your voice.

Our respiratory therapists help address your needs relating to cardiopulmonary health and respiratory function. They are vital for the ventilator-dependent client or any client with an open airway.

With support from the knowledge, expertise, and network of Dr. Anna Elmers, Designer Health Partners is able to offer unparalleled coordination of care in the comfort of a client’s home with various providers in not only Atlanta, but the entire state.

Upon initial engagement with DHP, you will be assigned a Case Manager who will complete an in-home evaluation and assess you and your family goals in areas including durable medical equipment (DME), transportation needs, home modifications, health and rehabilitation concerns, local/state/federal resources, potential referrals, self perception and insight, medications, and support system. Your DHP Case Manager will act as liaison to you and all parties involved.

What a client consumes is essential to the healing process. Our Dietician assesses the client’s needs relating to diet and nutrition and makes recommendations to optimize the client’s overall health.

Once a client has been evaluated by our team of neurorehabilitation therapists, they may recommend a consultation from our in-house AT Rehab Engineer. The AT Rehab Engineer assesses a client's needs related to the following areas of care: computer and cellphone access, adaptive video gaming setup, smart home setup, and custom AT device development. Through various mediums of product development, our AT Rehab Engineer is able to create custom made devices specific to the client’s needs, often not yet on the market.

As an integral part of the interdisciplinary team, the Counselor focuses on adjustment in the transition from hospital to home. This is accomplished through various settings including one-on-one with the client, family counseling, couples counseling, parent counseling, etc.

This four step process consisting of proper skin care, manual lymph therapy or drainage, compression through bandaging and/or garments, and a specialized exercise program are all specific care needs for clients experiencing lymphedema swelling following their Spinal Cord Injuries. Our lymphedema-certified therapists are able to bring this specialized care into the client's home as well as assist in arranging custom garment fittings and the creation of a tailored HEP for long term lymphedema management.

For clients who would benefit from massage therapy working in conjunction with our physical, occupational and speech therapists, a referral to our in-house massage therapist may be made. The massage therapist is able to target specific muscle groups with clients experiencing chronic pain, thereby allowing our neurorehabilitation therapists to focus their treatment sessions on addressing skilled discipline specific goals.

Once a client has been deemed appropriate for electrical stimulation for wound healing by a physician, DHP will engage our PT/PTA to perform high-volt electrical stimulation to prevent further wound damage and facilitate wound healing in an effort to avoid surgical intervention.

Wound, Ostomy, and Continence Nurses (WOCNs) are registered nurses with advanced specialty training in the assessment and management of complex wounds, bowel and bladder continence concerns, and ostomies. Using evidence-based practices, they help promote healing, prevent complications, improve quality of life, and provide education that empowers clients and caregivers to confidently manage care at home.
Designer Health Partners is proud to offer specialized WOCN services as part of our interdisciplinary care model. Through comprehensive evaluations, personalized treatment recommendations, caregiver training, and ongoing clinical support, our certified WOCN works alongside our therapy and nursing teams to optimize healing, preserve independence, and enhance long-term outcomes for clients recovering from neurological injuries and other complex medical conditions.

Designer Health Partners offers in-home Seating & Mobility services to help clients achieve optimal positioning, comfort, safety, and independence. Our certified Seating & Mobility specialists provide comprehensive wheelchair and seating evaluations, equipment recommendations, adjustments, education, and ongoing support while collaborating closely with physicians, therapists, vendors, and other members of the care team.
By integrating Seating & Mobility services into our interdisciplinary approach, we help identify concerns early, streamline equipment recommendations, and ensure each client receives the most appropriate mobility solutions to support their functional goals.
