Our services are appropriate for clients transitioning home from inpatient or outpatient rehabilitation, those requiring long-term neurological management, and individuals seeking to maximize independence and quality of life.

Unlike traditional home health, our care is not limited by insurance visit caps or rigid treatment schedules. We provide highly specialized neurological rehabilitation delivered by experienced clinicians who collaborate as one interdisciplinary team. Care is individualized, flexible, and designed around each client's long-term goals—not insurance limitations.

As stated in our name, we allow our clients to design their own healthcare program based on their goals and needs. Following initial evaluations from our expert team, we work directly with the client and the associated parties to design a healthcare program that best addresses both the short and long term goals of the client.

Yes, through our Case Management Services, we are happy to aide in obtaining physicians orders from your physiatrist or other primary prescriber.

We offer comprehensive case management services to any client to help navigate the health care system and manage ongoing care. We’re happy to discuss your needs and make recommendations if case management services would be a right fit for you. For clients engaging in two or more services, we do require assignment of a case manager to optimize and streamline services.

Because we build each client's interdisciplinary team specifically around their needs, onboarding generally takes 3–6 weeks. During this time we coordinate physician orders, scheduling, staffing, equipment needs, and individualized treatment planning so services can begin smoothly.

Yes. Designer Health Partners offers fully furnished, private, accessible homes designed specifically to support individuals recovering from neurological injuries and other medically complex conditions. Our rehabilitation-focused homes feature accessible bathrooms, therapy-ready spaces, safety modifications, and convenient locations near leading rehabilitation hospitals, allowing clients to continue making progress in a safe, supportive environment.
For clients traveling to the Atlanta area for care, we also maintain relationships with a variety of accessible housing providers offering accommodations at different price points and levels of accessibility. If you choose to utilize one of the housing options available through Designer Health Partners, your housing arrangements will be incorporated into your DHP Letter of Engagement. To maintain residency in a DHP home, clients must actively participate in at least two Designer Health Partners therapy services throughout their stay.

Designer Health Partners proudly serves clients throughout Georgia. Although most of our services are provided within the Metro Atlanta area, our team of specialized clinicians is available to travel statewide based on client needs. We also offer virtual consultations, when clinically appropriate, to enhance accessibility and ensure continuity of care regardless of location.

Therapy is staffed based on the frequency of a client’s needs and the location of the client. The team will consist of 1-3 therapists per discipline. This model facilitates an exchange of ideas and different treatment perspectives.

Every client's rehabilitation journey is unique, which is why there is no one-size-fits-all therapy schedule. Following your initial evaluations, our interdisciplinary team will develop an individualized plan of care based on your diagnosis, functional goals, physician recommendations, endurance, and overall medical needs. Therapy may range from multiple sessions per week to a maintenance program focused on preserving function and preventing decline. Your treatment frequency is continuously reassessed and adjusted as you progress to ensure you receive the right level of support at every stage of recovery.

Upon the conclusion of initial evaluations, our therapy team will determine an anticipated length of care based on your functional goals. When goals are achieved and independence grows, we celebrate success! Discharge marks a transition, but we stay a resource to you if needs arise. Some clients benefit from ongoing care or support to maintain progress, adapt to changes, or pursue new goals. We offer flexible PRN-based services to stay responsive to evolving needs.

For the convenience of our clients, we utilize Google calendar to communicate with all involved parties. Through the client Google calendar, our team is able to ensure seamless communication with the client and all involved parties. With the use of the Google calendar app, you will be able to access daily, weekly, and monthly appointments as they change in real time. Google calendar also allows the client or caregiver to organize personal appointments. Our team works very hard to schedule therapy in a time convenient for you. Working around the client schedule, our team can schedule therapy in the morning, late afternoon, evening, and on both weekdays and weekends.

Service costs vary based on the location of the client, projected length of service time, and complexity of care. We work very hard to tailor each contract to meet the financial budget set in place by the client or those caring for the client.

Designer Health Partners is a private-pay practice and does not bill Medicare, Medicaid, or commercial insurance plans directly. This allows us to provide truly individualized, client-centered care without the restrictions often imposed by insurance companies, such as visit limits, pre-authorizations, treatment caps, or discipline-specific limitations.
Our team designs each plan of care based on your unique goals, clinical needs, and physician recommendations—not insurance guidelines. This flexibility allows us to coordinate multiple disciplines, adjust the frequency and duration of treatment as your needs evolve, provide longer or more intensive sessions when appropriate, and deliver comprehensive care that supports long-term recovery rather than short-term coverage.
We also work closely with workers' compensation carriers, case managers, attorneys, trusts, and other third-party payors when applicable to coordinate payment and authorization for services.

Yes, upon request, all submitted daily and progress notes can be accessed on our EMR client portal. Please note for this option, only clients or identified caregivers can access the client portal. Alternatively, all submitted documentation can be sent via email to any requested parties by your DHP Case Manager at the end of each month to stay up to date on all the client care.

No. Designer Health Partners maintains a wide variety of portable neurorehabilitation equipment that our clinicians bring directly into the home as needed to support your treatment plan. This allows many clients to begin therapy without having to purchase specialized equipment upfront.
If one of our therapists determines that durable medical equipment (DME) or other specialized equipment would benefit your safety, independence, or long-term rehabilitation, your DHP Case Manager will work with you, your physician, and the appropriate vendors to coordinate the evaluation, ordering, and acquisition process. Our goal is to ensure you have access to the equipment you need while making the process as seamless as possible.

DHP therapists are skilled in tailoring each session to your specific needs. After a thorough initial evaluation, they will collaborate with you, and family or care team as applicable, to determine your goals. From your goals, they will break down each component or skill needed and cater each session to building your individual rehabilitation program. Our therapists collaborate frequently together, and may even complete co-treatment sessions to further maximize potential of our in-home sessions together.

Every therapy session is individualized to your goals, abilities, and stage of recovery—no two sessions are exactly alike. Your therapist will develop each visit around meaningful activities that help improve your independence, safety, and quality of life while addressing the challenges that matter most to you.
Depending on your needs, sessions may include practicing functional mobility and transfers (such as getting in and out of bed, a wheelchair, or a bathtub), improving strength, balance, coordination, and endurance, managing muscle tightness through specialized techniques such as dry needling or other therapeutic modalities, developing cognitive and memory strategies to increase independence with daily tasks like medication management, or practicing activities that support returning to work, school, hobbies, or community participation.
Because we specialize in neurological rehabilitation, our clinicians think beyond traditional therapy approaches. We use creativity, evidence-based interventions, and real-life activities within your own home environment to make every session practical, engaging, and directly applicable to your everyday life.
