Hi! I’m Dr. Anna Elmers
J.D., M.D., a board-certified physical medicine and rehabilitation physician, and founder of Designer Health Partners.
My experience in post-acute neurorehabilitation inspired the creation of Designer Health Partners. After years of working alongside individuals recovering from complex neurological injuries, I recognized a significant gap between traditional healthcare services and the comprehensive, coordinated support clients truly need once they return home.
Designer Health Partners was founded to bridge that gap. By bringing together rehabilitation specialists, case managers, caregivers, and care coordination under one integrated team, we provide a seamless continuum of care designed to maximize independence, improve outcomes, and simplify the rehabilitation journey for clients and their families.
Our mission is to redefine post-acute rehabilitation by delivering personalized, interdisciplinary care that meets clients where they are—empowering them to achieve meaningful progress in the comfort of their own homes and communities.

Anna Choo Elmers, M.D., J.D., is a board-certified PM&R physician with additional subspecialty board certification in brain injury medicine.
Prior to medical school, she practiced law for a brief time after obtaining her Juris Doctorate from the University of San Diego, School of Law.
She moved to Atlanta after completing medical school at the George Washington University School of Medicine in Washington, D.C. to complete her residency in physical medicine and rehabilitation at Emory University. Following residency, she had the privilege of joining the staff at Shepherd Center, where she gained invaluable experience in the care of the catastrophically injured patient.

Dr. Elmers maintains a very busy home life with her husband, three daughters, and three rescued dogs.
